Sometimes, when things are slow, you might find some strange patients coming through the Animal Hospital doors. You can tend to them or choose to turn them away. One patient you shouldn’t ignore is Barney, so keep your eyes out for him when he does appear randomly.

Barney Anomaly Explained
Barney will appear 5 different times after Shift 2. During my first playthrough, I spotted an unusual white rabbit creature with a black mustache and a brown hat appear during Shift 4. At first, there wasn’t anything unusual about him, and he appeared like any other customer.
But he does have an unusual request. He will stop by the counter and ask for a cup of coffee. Hand him a cup while having one yourself to keep your sanity in check.
Second Barney Encounter
The next time I encountered Barney, he asked for another cup of coffee. He appeared during Shift 6 for me, but it could be any other day, too. Again, repeat the coffee process, and he will give you a briefcase. The briefcase will be stowed away under your desk, and you can’t open it for the time being. Continue with your day and attend to your patients in the meantime.
Third Barney Encounter
After handing Barney his coffee a third time, he will ask you to photograph him to reveal his true identity. This is also when it is revealed that he is an anomaly, but it is too late to close the shutters on him now. After he leaves, continue your day as normal.
Fourth Barney Encounter
While asking for coffee was strange, Barney’s fourth request is even darker. He will ask for a scalpel, and he wouldn’t leave until I handed him one. There is a scalpel in the surgery room down the left hall, in Room 8. He will leave, but not before taking his suitcase back with him.
Fifth Barney Encounter
The last time Barney appears, he won’t say anything. Interact with him to get the option to hide him. After a while, Officer Duckman will appear and ask him if you had seen a serial killer on the loose. He will also show you a photograph of Barney. You have two options here: either stay quiet until the timer runs out or snitch and tell the officer about Barney.
Hide Barney or Hand Him Over
The best choice here is to stay quiet, as helping Barney will reward you with an extra coffee machine. This is extremely useful for improving sanity regeneration. Once the timer runs out and after the officer leaves, Barney will give you the suitcase. Check the cabinet with the old coffee maker to find the new one beside it.
If you refuse to help Barney and turn him in, he will run out of your cabin and stab the officer before running away. You now have an extra patient to treat, so carry him to the designated room to heal him. Barney will still be on the loose after that, but he will not appear at your hospital ever again.
