Currently, the way to travel between these two maps is with the help of an NPC sorcerer called the Adherent of the Outer Darks. These NPCs will be available in 3 locations in each map, serving as fast travel points to the other map.
Exiled Lands to Isle of Siptah Fast Travel Location # 1 | Conan’s Tavern
Conan’s Tavern in the Exiled Lands is a place many players revisit often. And just outside to the right of the tavern’s entrance is a sorcerer who can take you to the Isle of Siptah.
Exiled Lands to Isle of Siptah Fast Travel Location # 2 | Shattered Bridge
If you are near the Shattered Bridge location, fast-travel to the Isle of Siptah is more convenient from the sorcerer marked in the location above. It will be present next to the shore with a bonfire lit next to it.
Exiled Lands to Isle of Siptah Fast Travel Location # 3 | Narrowneck Span
For the players near the Narrowneck Span, the above-marked fast travel location works best. However, you would have to swim a bit to reach this sorcerer. The sorcerer will be present again next to the shore with a bonfire lit.
Isle of Siptah to Exiled Lands Fast Travel Location # 1 | Siptah’s Tavern
Like in Exiled Lands, Siptah also has a fast travel option near its tavern. But the fast travel sorcerer for Siptah’s Tavern is present inside it, just to the right of the bar counter. The sorcerer will be standing next to a big blue vase, which can serve as its identifier.
Isle of Siptah to Exiled Lands Fast Travel Location # 2 | Tears of Xanarus
If you are near the left edge of the Isle of Siptah, this fast travel will be the closest to you. You need to head over to the location marked in the image above, between two giant structures dividing the shores. Once here, you need to climb on top of the structure, which will have a bonfire lit. Next to the bonfire, you will find the sorcerer.
Isle of Siptah to Exiled Lands Fast Travel Location # 3 | Scavenger Coast
If you are near the right edge of the Isle of Siptah, this fast travel will be the closest to you. Head over to the marked location in Scavenger Coast. At the location, you will find one ruin with one intact and one collapsed tower. Head over inside the ruin, and you will find a sorcerer inside.
