WoW Forever does have some repeated quests, but at the same time you will also encounter other special quests that start after picking up certain items. If you had the Abominable Head sitting in your inventory for quite some time, then you need to turn it in to a certain vendor to kickstart the Unending Torment quest.

Unending Torment Quest Part 1
The quest does not start through any NPC. Rather, you need to pick up the Abominable Head by slaying the Baron. This boss is found in the Ruins of Lordaeron, which is a 5-player dungeon located above Undercity. You need to be level 15 and above to access the dungeon, and I recommend teaming up with other high-level players to make the trip easier.

After obtaining the Abominable Head, check your quest log to find the Unending Torment quest. If it doesn’t start, then inspect the item in your inventory to get the quest. The quest objective will state that you need to visit someone in the Undercity. Make your way to the Apothecarium and find Faranell in the lower sections of the western part of the building. The NPC will have a ? quest marker on them, so you can’t miss them. Speak to them to turn in the Abominable Head and start the next quest.
Reward: 5,300 XP
Unending Torment Quest Part 2

Faranell would be thrilled to get the head, and would like it to speak so he can learn all of the Baron’s secrets. He will instruct you to place the head in the next room where the body of Othmar is. The room in question is to the north of your previous location, and the entrance is right behind Faranell. Inside this room is a hanging body of a giant. Interact with it to place the head on it and get the next quest.
Rewards: 13 Silver, 1,650 XP
Unending Torment Quest Part 3
Visit Master Apothecary Faranell again and report your findings to him. This step is quite simple, and you won’t have a hard time completing it. Once this quest objective ends, you can pick up one of the following as your reward:

Unending Torment Quest Part 4

Faranell has other plans now that the head and the body can react to external stimuli. He wishes to bind it to the Dark Lady, but he requires a potion to make it work. For the potion’s ingredients, you need to collect the following:
- Toxic Skullcap
- Blisterweed
- Essence of Agony
Luckily, you don’t need to go too far to find the required vendors who will sell these items to you. Visit the Rogues Quarters to find Ezekiel and purchase the Essence of Agony. Then, head back to the Apothecarium to find Blisterwood at Martha and Katrina Alliestar’s Herbalism store. Then, make your way up to the Trade Quarter and speak to Tawny to get the Toxic Skullcap. Return to Faranell to complete the quest objective.

Unending Torment Quest Part 5
The final part of the questline is to inject the newly created Hissing Serum into Othmar’s body. You will get the Hissing Serum from Faranell, and all you need to do is make your way north of Faranell’s location to find the giant’s body. Interact with the body to administer the serum and complete the final quest.

Why can’t I pick up this quest?
You need to be level 16 to enter the dungeon and pick up the Abominable Head, and the rest of the quest unlocks at level 21.
How do I beat the Baron?
The easiest way to beat the Baron is to team up and let the other players attack it, then collect the head for the quest.
Martha doesn’t have the Blisterwood in the store menu; what do I do?
You don’t need to interact with any of the store assistants to collect the Blisterwood item, as you will find it in a large vase in the corner of the shop. You can walk up to the vase and interact with it to pick it up.
