Nameless Hospital Quest Guide
The Nameless Hospital anomaly is found in Illusion Town, which unlocks as you progress the main story. Once accessible, head to Wertheimer Tower, then check the Anomaly Commission tab for the exact hospital location.
Enter the tower through the main entryway and explore the lobby until you find an exit that leads to several flights of stairs. Over here, you will find a person facing the wall. Speak to the person and select the Close Eyes dialog option. Your next task is to locate several clues around the new area.
Anomaly Disorder Clue #1
The first clue is the easiest to find, as you just need to hop over the desk on your right and interact with the computer to get the clue. If you are confused about where to go next, look around for indicators pointing you in the right direction.
I found out that sightings of entities going through doors or looking around for water droplets helped me figure out where to go next. You could also check out the walls for directions.
Keychain 1F Location
Exit out the left doorway and turn right on the corridor. You will find an entity entering one of the doorways on the right. Enter that room and go around the obstacles until you find another door. Go through that one to reach the locker room area. Explore the locker room to find another door on the far right corner.
Continue to go through this room and exit through the doorway on the left. Turn left again and enter the doorway on the right. Go left again and enter the door on the right to get inside a processor room. Activate all the inactive processors until the entity enters the room. Exit out the open doorway and get behind the desk to collect Keychain 1F.
Anomaly Disorder Clue #2
Exit out the door on the far left side of the room and keep going straight past the other exit point. Do not go left or right on the crossroad, go straight some more, and enter the first door on your right. You will reach an operating theater. Interact with the computer to get the second clue.
Anomaly Disorder Clue #3
Use the door on your immediate left and exit to the next room. Turn right and take the left side exit in the corner. You will reach another operating theater, turn to your left, and use the large exit. Continue straight ahead until you get to a dead end. Go through the door on your left and interact with the third PC to get the next clue.
Spare Key 1F Location
Exit the room and go past the second room, too. In the third room, investigate the lockers on the left to get Spare Key 1F.
Place All Clues
Use the door beside the lockers and run to the left door just adjacent to the one you exited from. Head up just one flight of stairs and use the door behind the desk. You will find three video cameras in the room. Interact with each one and place all three clues in it by selecting the View Clue and Place Clue options. Watch the sequence that unfolds to complete the quest.

How do I unlock the other 3 endings?
Once you have unlocked 1 ending, you will get the Hospital Staff Elevator card. Use this card on the elevator opposite the doorway where the entity enters the room (after finding clue #1 and on the way to find Keychain 1F Location). This will take you to the upper floors, where you need to find three clues of the same type and use them on the video recorder to unlock the other endings.
What items do I need to unlock the other endings?
For ending 2, you need to locate all 3 Surveillance Record clues. For ending 3, you need to find all 3 Anomaly Experiment clues. Ending 4 unlocks after finding all 3 Nightmare Voice clues.
