You can entreat the Forbidden Tome from the Muttering Codger NPC in Flamesgrace, Frostlands Region.

Head to the Victors Hollow: Old Quarter, located in the Woodlands region. Now, cross the wooden bridge, and you will come across the Old Story Teller NPC. Talk to him and entreat the Forbidden Greatbow.

Get to Sufrataljah in the Sunlands region, now hunt down the NPC Taciturn Housewife, located inside the house south of the area. You can purchase the Forbidden Dagger from her.

#4. Forbidden Spear | Grandport
Get to Grandport Markets, Coastlands Region, and you will find Sea-Gazing Graybear, left of the colosseum in the area. You can buy the Forbidden Spear from him.

This one takes us to Atlasdam in the Flatlands Region. Talk to the NPC named Once-Famous Actress, and you can buy the Forbidden Fan from her.

You will need the equipment or amulet Blessing in Disguise to counter the negative effects on all Forbidden items. By wearing the amulet, you can turn any negative value of any number to a positive stat. You can get the amulet from a shop in Wishvale.
Are Forbidden Items any good in the game?
Yes, the stat and the special effects make them the best choice, and once you add in the Blessing in Disguise amulet to the mix, the whole deal gets better.
Are any of the Forbidden Weapons or Shield missable?
None, you can get them at any point in the game.
